This lovely gentleman today presented with an infected root.

This is an upper molar. You have the old amalgam that’s dislodged from the cavity and now pushed into the gum. He has an infection at the end of the root.

The tooth requires extraction, and this is the condition of the tooth. As you can see, the amalgam was shoved into the gum, which was causing him pain.

It was a simple procedure. Our patient did awesomely and is the hero of the day!

A 94-year-old man who did wonderfully!!

Hi everyone, good afternoon, I like to share the story of this tooth. This little boy, five years old, came to my office this afternoon and unfortunately, this little boy has not been comfortable when taking bitewing X-rays, which are our checkup X-rays. Today, after a while, he came in and I noticed in my clinical exam that he had developed decay, and his mother was also complaining that the child is having, experiencing pain when eating. After I anesthetized him, I was able to detect another cavity adjacent to the other tooth. So, he needed to cavity fill and that is what I wanted to emphasize, that you need to bring your child to the dentist for preventive care. And that is how we can catch cavities as such, preventing them from getting into bigger, larger sizes and causing pain and discomfort. So, the emphasis is pediatric dentistry is valuable and it’s amazing for the future to have really healthy and good teeth. He was happy, I’m happy, and we accomplished what we needed to do.

Hello everyone, today’s case was actually interesting – this patient has presented with the missing permanent bicuspid – so this patient is in her 40s and is still holding on to her baby tooth because her permanent bicuspid was missing from birth and this patient had a very extensive build up on this tooth for a long time and eventually the buildup has broken down so today’s conversation we had – what are we going to do with this?
Shall we go ahead – one of the options we discussed was to go ahead and extract the tooth, and, have an implant placed in and get her a crown over the implant, but that was not, unfortunately, part of her budget so I decided that even though the tooth is really compromised, I cannot even put a crown on it, and I decided I can do another extensive core buildup and by doing that this is how the tooth presented and the filling and portion of the tooth was lost, and I ended up to build up all over again and this is one of those cases – its unpredictable and I already informed the patient this tooth, the core build up may not last because of the poor prognosis of the tooth, eventually believe it or not – this tooth is going to get mobile and it’s going to be lost just due to the fact that the tooth has no roots in the bone and eventually she’s going to need that implant, but for the time being this was a perfect cure for her condition. I am glad to share it with you, let’s hope it’s going to last her for a long time.

Hi there, I am Dr. Roya Shoffet, and my office is in West Hills Los Angeles, CA. I like to bring you the journey of this tooth that we treated for this lovely patient of ours.
He had bad decay on one of his teeth and needed a crown. As you can see, he has a missing tooth area for that he needed an implant.
On this X-ray, you see our specialist periodontist placed the implant and after the period of healing took place, here is the date of delivery.
We have two beautiful crowns, one is a porcelain crown that was treated for the decayed tooth, and this is the crown with the screw-retained.
It has a temporary filling right now, and later on, we’re going to do the porcelain repair, place composite and it’s going to be invisible.
He’s happy. It was an excellent result and a journey we accomplished for this patient.

Hi everyone, our beautiful patient today had an emergency. She has a 5 unit bridge where the porcelain fractured at the pontic area, and it got into two pieces.

The patient tried to fix it by herself, and obviously, it didn’t work. Today, we brought her in and did the emergency repair.

This is what we call a porcelain repair, and I’m still in the process of polishing it. As you can see, we bonded the fractured piece, and she’s all fixed together.

She can have her beautiful smile back until we do the plain old treatment, which could be a brand new bridge.

Unfortunately biting into a cherry seed she actually broke the tooth. And on this tooth we decided that the final restoration would be a full covered crown. On the day of delivery of the crown, this is the final picture. The tooth is already restored and it looks beautiful, and the patient was very happy with the result.

Hello everyone, today’s case I want to present to you is a gentleman who is a great patient of ours. Last week I noticed that he has an old amalgam and he has a recurrent decay at the margins of the amalgam, and there was a crack line. Apparently, he bit into something really hard and was experiencing pain upon biting. That is a sign of cracked tooth syndrome. My concern was that we would need to make a full coverage crown and possibly a crown lengthening gum surgery because his bite is really deep. So he would have needed extensive and costly treatments. This is where you can see the recurrent decay. Today, I removed all the old filling and decay and discovered I can do what we call a porcelain onlay. Onlay meaning the weak points of the tooth. These two walls of the tooth that I needed; are what we call the chewing cusps. By eliminating the risk of fracture on this tooth. It’s going to be an extensive onlay. I eliminated the crack line present at the facial surface of the tooth, I did some build-up here to create a smooth surface for the onlay, and we are ready to take impressions and send them to the lab. I saved him from all those unnecessary treatments, and porcelain onlay is going to be the magic of the day. He’s doing wonderfully, and we’re ready to take the impressions. Hello everyone, today’s case is our new bride to be getting professional whitening before the wonderful event, and today we’re using the Ultradent Boost. It’s fairly easy applying the resin to the gum to protect the gum, and then apply the bleaching material to the teeth. We have already prepared her. She has used fluoride prior to the visit, and we have used some boost, and Ulradent Go bleaching material to perk the process. I’m sure we’re going to have a super white and beautiful bride to be with a beautiful smile. It’s going to be 15 minutes, I’ve applied it again. She was undergoing orthodontia, so I believe she developed this decay due to some food debris collecting at the gum line.
Luckily she was here soon enough, and we did the bonding, provided composite filling, and restored her cavities.
This is immediately after, so the gum is irritated. But she’s happy that we got it right away, and it’s beautiful all over again.

Hello, everyone. Today, our lovely patient is presented with discomfort and pain at the gingival portion of this tooth.

It was a decoy, and I didn’t know why this tooth could have had so much pain. By retracting the gum, I realized that there was a serious abrasion at the neck of the tooth.

We call it Cervical Erosion.

She had sensitivity to cold. After I had prepped the tooth, many steps and precautions were taken because there is hardly an enamel at the cervical portion of the tooth.

Here is the final product. We filled it, and now she’s out of pain and discomfort—an easy fix to a very painful condition.

Happy Friday everyone. Today we had a lovely day at Royal Dental Practice. We have cemented and delivered a crown and an olay, and I want to share it with you.

This is how the tooth started day one, and the cavities were underneath the old amalgam fillings, and the patient had crack lines and all sort of a problem with it. We have prepared it, this a

model that you can actually see the model is prepared. Today is when we wanted to deliver it. We prepared the tooth, and it’s all in rubber – it’s all protected and this is the final result. If you have one of those ugly looking feelings which is probably 100 years old and nobody has ever attended to it, just give us a call i can make them look like new, and make them brand new too. It is unbelievable the result you cannot even detect the margins of the tooth previously versus the tooth now, and hopefully it will be a very successful and permanent restoration for this patient. Dr. Shoffet: Good afternoon everyone. Today is the first case of the wisdom tooth impaction extraction of 2024 hippie. So it’s an interesting case. Once I was describing the consent form for our lovely patient, I mentioned to her I could see a little tiny curvature at the end of that root and I mentioned that’s one of the risk if it gets separated we may leave it because of the anatomical proximity to the mandibular nerve. And here it is, we extracted the tooth and by looking at it very carefully, that tiny, itsy bitsy tip was separated exactly as I explained to the patient and I took an X-ray and I could see it yes. And sometimes believe it or not, if it is, could be a risk to our damaging our mandibular nerve we may leave it there, but carefully, very carefully I approached it with my great assistant and cooperation of our lovely patient and look, I could remove with a little root tip that was removed and I’m very happy we stitched her it’s all lovely. We extracted her upper tooth as well and today is double tooth extraction of the wisdom tooth. Our patient did lovely and I’m very happy.

Hello everyone. This is Dr. Roya Shoffet. I’d like to share this case that I had to treat recently for this patient. This patient presented with excruciating pain and swelling on her lower left molar, and she has neglected her teeth for a long period of time. And as you can see, the condition of this tooth is horrible and also we see a decay in the next door tooth, and I’d like to share it with you. So this is kind of a red flag for people who go without treatment for a long time. Obviously after X-ray, this is the radiographic image that you can see. She has severe infection between her roots, which explains to me that this is part of the reason that she has fractured root. She has decay on the next door tooth, adjacent tooth, and obviously this tooth is not saveable nor restorable. We need a extraction and requires surgery; we splitted the roots. As you can see, it came out in pieces. The root was fractured. So the lesson from this case is this patient received a root canal at some point in her life, but she never followed up with the treatment. And as you can see, she had some temporary filling that fell off and she never attended to the final treatment. So as you can see, she is gonna lose, practically this tooth is gone. And also this one will require severe, you know, extensive treatment, possible root canal, gum surgery if we need to, and full coverage crown and the tooth was splitted so we needed to extract it. And after extraction, hopefully in a few months, we have applied bone graft to it. And after a few months, she’s ready to receive an implant. With our marvelous Miss Christina, she could get her financing and she could do the treatment. She was always worried, and by a few of these magical helps, she got her treatment done. I’m happy for her. And I just wanted you to get a lesson for those who are neglecting or postponing treatment.

Today’s case is an interesting one and, I would like to share it with you. This is how our lovely patient is presented today.

Clinically, we see a deeper grove at the palatal side of these two teeth: the two lateral incisors, and interestingly enough, if we take a look at the X-Ray, you can see an interesting situation.

A portion of the pulp may be traveling towards the tooth’s pits, which could create a problem in the future for the patient if left alone, as it could lead to some lesion at the end of the root.

Thankfully, there is no lesion at the end of the root currently, and the patient has no symptoms.

So we ended up prepping the tooth from the back, cleaning it up, putting on some medication, and doing some composite fillings.

And this is the final result.

Hello everyone, I’m Dr. Roya Shoffet. I’m talking to you from West Hills in California today. I have a patient & I like to share with you about the Journey of this patient that we started actually as of last week, and I want to show you how he presented to our office. Even though some of my followers always mention why I show the extreme, but, it’s very important for you to see what happens once we have a patient which I call, has Dental suicidal, meaning this patient has neglected his mouth for over 20 years. He was a business owner and you know employed 50 employees or more and he never cared about his teeth. And now in his late 70s, all of a sudden, he’s getting suffering by memory loss and is going through dementia, and the family realized all of a sudden that he has neglected oral health. Look how he presented to us. Number one – the gum condition. He has calculus buildup everywhere around his teeth, he has poor oral hygiene. This is what happens once you neglect your teeth and this is not for you to get grossed out. I just want you to see, you know share with you that once you neglect your oral health this is what happens. He had work done. He had crowns, but look at the margins of the crown, all of the margins are developing decay at the margins of their Felix margin of the crowns. And you know this is, look at this – this is this Crown is not you know any more restorable. We have to redo the crown. This requires major oral rehabilitation. We need to approach it first from obviously deep cleaning, soft tissue management, get rid of the calculus and all the inflammation of the gum. Then we come after the decay and all these broken teeth. Actually, his chief complaint was this tooth that was broken and he just said yeah my, the, pieces of my tooth are falling off, not knowing the rest of the mouth is also you know in this kind of a poor condition. And you know the fact of the matter remains, what do you think the dental IQ of this patient is? He practically didn’t respect his oral health at all and that’s why I want to bring across, once you neglect your oral health this is what we’re going to witness and this is not an overnight treatment. This patient will require gum surgery, will require root canal, restorative fillings, and a whole extensive detailed treatment is not going to happen overnight, and guess if this patient goes towards severe dementia, he’s going to turn uncooperative, and to treat a mouth like this then we need to put them on your anesthesia. You can see the vast extensive treatment that he’s going to need and the cost and the time and the pain and suffering the family has to go through. So, please let’s cheer for all the patients who have perfect Dental IQ. They care about their teeth. Today I wanted to share this you know, advanced situation of dental suicidal. I call it, this patient had no value for his teeth. And stay tuned, I’ll discuss the course of the treatment.

Good Afternoon everybody. My name is Dr Roya Shoffet and I am a dentist, and I practice in West Hills. My passion has always been for patients with Alzheimer’s and dementia. This morning I attended a gathering at the San Fernando location for the Alzheimer Association. And I’m very proud to be joining this group of wonderful people who really cater for our elderly in the community, who normally are helpless about their dental health, because most of the time these patients are not able to be seen by a regular dentist who has no experience. So throughout the years in my practice, I have always been an advocate, and alzheimers has been very close to my heart, and I try to help them as much as we can, because we know as a fact that dental health plays a major role in everybody’s medical health, and any inflammatory process that is going to trigger that cascade – we want to prevent that, and that could start with the bacteria in the mouth. And that’s the reason I’ve always been a resource for all the families, for all the assisted living, and I’ve been privileged to help them to take care of their dental health. And remember it is very easy to be forgotten once patients or admitted in a facility, and the dental health just goes under the carpet, and nobody really thinks about it. And I want to bring awareness for our community so hopefully we can prevent them from worsening their Alzheimer’s, deepening their dementia and all their medical health could be affected – high blood pressure. diabetes – these all and bacteria starts from the mouth. And remember, if we keep them very well-maintained with dental health, their general health is going to be improved. So I want to bring out this message. Let’s join together to fight alzheimers. There are always a number of walks to end Alzheimer’s and I would love to attend as much as I can, and contribute to that cause, because as I said, it is very close to my heart.
