- Specialty care (respiratory therapist and/or registered nurse) equipment and personnel for up to three hours of transport time – $3,659 to $4,4143
- Hourly rate after three hours for registered nurse and/or respiratory therapist specialty care – $206
- Disposable medical supplies – $33
