For starters, Dr. Shoffet works with patients to avoid the damage that leads to the need for restorative treatments. Tooth removal is largely a “last resort” procedure. Many oral conditions leading to tooth loss are highly preventable and progressive; they may be stopped with prompt and appropriate intervention. For teeth that cannot be saved with interventions or procedures like root canal therapy, Dr. Shoffet will largely recommend the removal of the tooth followed by replacement with a bridge, denture, or implant as needed.
Wisdom teeth are the exception to the “rule” about tooth replacement. Like the appendix and tailbone, wisdom teeth (third molars) are vestigial structures. They are not necessary for oral health and function. So they do not need to be replaced. These molars need to be monitored closely to ensure they are developing properly. Complications such as impaction also require removing or extracting the affected teeth.
Situations such as impaction generally require surgical extraction. When a tooth is impacted, it becomes partially or fully trapped in the bone underneath the gums. Unlike simple extractions, which involve loosening and lifting a visible and intact tooth from its socket using special instruments, surgical techniques involve first accessing the tooth by opening up the treatment site. Once the tooth is accessed surgically, it can then be removed. This latter approach may be appropriate for any tooth that is not intact and cannot be easily accessed with a simple or common extraction procedure.
